Full Time Services:
Conducts risk assessments on all new admissions and re-admissions
Follow the PAR program protocol
Conducts daily rounds to identify and address issues with a timely response
Supports the delivery of more complex care
Provide appropriate PDPM diagnoses when applicable
Enhance staff education through in-service and on-site training
Increase the level of resident and family satisfaction
